What is the difeerence between CREATED, MODIFIED, and ACCESSED in Spotfire database table LIB_ITEMS

I'm looking to define which dxps haven't been touched in the last 2 years, my original query looked at LIB_ITEMS.ACCESSED for the dxp ITEM_TYPE but I noticed sometimes it's null. I also noticed sometimes LIB_ITEMS.MODIFIED>LIB_ITEMS.ACCESSED. What does that mean How could a dxp never have been accessed or how could a dxp be be modified more recently than it was accessed
